Breast cancer is the most common malignant tumour in women and was formerly treated by total mastectomy. Nowadays in most situations of breast cancer mastectomy can be avoided. However, in some clinical situations by medical reasons a mastectomy still has to be performed. The immediate euphoria of having survived the primary treatment in many cases slowly leads into a period of depression with feelings to be mutilated and no longer a woman.

In this situation breast reconstruction can improve the quality of life supporting selfconfidence and the emotional status. For breast reconstruction a number of techniques have been developed. In our institution we pefer the secundary reconstruction with alloplastic material in two operation steps. In the primary operation a balloon-expander is inserted and  during the following weeks filled with saline solution.

In this first operation additionally the remaining breast undergoes a reduction / lift to adjust the breast silhouette. The balloon-expander creates the pocket for the definitive implant which is inserted after 10 – 12 weeks.

In this second operation the surgeon, if necessary, can perform corrections of the implant pocket for a better adjustment. For completion the nipple-areola complex is reconstructed with the patient`s own tissue. If the surgeon is familiar with these techniques this is a simple and low risk procedure with a good standardization.
